About approval of document forms on rendering the state support of insurance of agricultural products

1. Approve the following forms which are applied:

1) requests for obtaining in 20 ___ to year of compensation of insurance payment (award);

2) statements for inclusion in the list of the insurers having the right to insurance of agricultural products with the state support;

3) the list of insurers who have the right in 20 ___ to year to receipt of compensation of part of the paid insurance payments (premiums) to insurance contracts of agricultural products with the state support;

4) information on each prisoner extended and the stopped insurance contract of agricultural products with the state support in 20 ___ to year;

5) information on each case of payment of insurance indemnity or refusal in payment of insurance indemnity for the signed insurance contracts of agricultural products with the state support in 20 ___ to year.

2. Provide to finance and economics department in accordance with the established procedure provision of this order on state registration in the Ministry of Justice of Ukraine.
