1. Approve enclosed:

form of the act of carrying out unscheduled inspection of observance of requirements of the legislation on electronic communications and radio-frequency range;

form of the act of factual determination of illegal transfer or provision to use of individual rights to use of radio-frequency range to other legal entity or physical person;

form of the act of repeated unmotivated refusal of the user of radio-frequency range in conducting check by regulatory body for compliance with law concerning use of radio-frequency range;

form of the act of further violation by the user of radio-frequency range of conditions of the license and/or other obligations, the stipulated in Article 49 Laws of Ukraine "About electronic communications";

form of the act of failure to meet requirements of the order of regulatory body;
