1. Approve:

1) the Standard form "Information on Subjects of the Taxation, Objects of Outdoor Advertising and Objects of Road Service, and also Trade and Economic Objects of Which Are Placed in Zone of the Public Road and/or the Conservation Zone, behind Line of Settlements (Forma IZDP 20)", according to appendix No. 1 to this order;

2) the Instruction about procedure for filling and representation "Information on subjects of the taxation, objects of outdoor advertizing and objects of road service, and also trade and economic objects of which are placed in zone of the public road and/or the conservation zone, behind line of settlements (Forma IZDP 20)", according to appendix No. 2 to this order;
