Approve the Procedure for carrying out by persons authorized by board of the Pension fund of Ukraine, checks of justification of issue and prolongation of the enclosed disability leaves.

Procedure for carrying out by persons authorized by board of the Pension fund of Ukraine, checks of justification of issue and prolongation of leaves of disability

1. This Procedure determines the procedure of carrying out by persons authorized by board of the Pension fund of Ukraine, checks of justification of issue and prolongation of leaves of disability and documents that is the basis for their forming (further - check of leaves of disability).

The purpose of check of leaves of disability is control of justification of forming (issue) of leaves of disability, medical certificates about temporary disability (further - medical certificates).

Leaves of disability are performed by authorized officers of territorial authority of the Pension fund of Ukraine and/or authorized doctors which list affirms board of the Pension fund of Ukraine.

Medical certificates are performed by exclusively authorized doctors.
