About approval of the Description, specification and sample of visa form

For the purpose of approval of the description, the specification and sample of visa form, the Cabinet of Ministers of the Azerbaijan Republic decides:

1. Approve "The description of the form of the visa" (appendix No. 1).

2. Approve "The specification of the form of the visa" (appendix No. 2).

2-1. Approve "Sample of visa form" (Appendix No. 3).

3. Charge to the Ministry of Finance of the Azerbaijan Republic to provide acceptance in stipulated by the legislation procedure for adequate measures to start production of the form of the visa according to its description and the specification.

4. This resolution becomes effective from the date of signing.

Description of the form of the visa

The description of the form of the visa on departure from the country and entry into the country issued to foreign citizens by embassies, consulates and other diplomatic representations of the Azerbaijan Republic in foreign countries is printed according to the machine-readable format of the visa (MRV-A) provided by part two of the document No. 9303 of International Civil Aviation Organization.

In the visa the following data are specified:

- the name of the Azerbaijan Republic in the Azerbaijani and English languages, in also word "VIZA", the series consisting of two Latin letters and seven-digit number of the form of the visa;
