Bring in the resolution of the Cabinet of Ministers of Ukraine of June 29, 2021 No. 677 "Some questions of forming and use of the certificate confirming vaccination against sharp respiratory disease of COVID-19, of SARS-CoV-2, caused by coronavirus negative result of testing or recovery of person from the specified disease" (The Official Bulletin of Ukraine, 2021, No. 54, the Art. 3342, No. 76, Art. 4771; 2022, No. 14, the Art. 776, No. 65, the Art. 3917, No. 98, the Art. 6118) the enclosed changes.

1. To exclude the subitem 3 of item 4 of the resolution.
2. According to the procedure of forming and use of the certificate confirming vaccination against sharp respiratory disease of COVID-19, of SARS-CoV-2, caused by coronavirus negative result of testing or recovery of person from the specified disease, approved by the specified resolution:
Item 5 to add 1) with the paragraph of the following content:
"COVID certificate can be created after entering into the central database of data on vaccination.";
2) in appendix to the Procedure:
in Item 1 of table 1 to state the column "clinical criteria of forming of COVID certificate" in the following edition:
