About modification of the procedures approved by resolutions of the Cabinet of Ministers of Ukraine of May 7, 2014 No. 152 and of March 25, 2015 No. 302

The Cabinet of Ministers of Ukraine decides:

1. Bring in the Procedure for registration, issue, exchange, transfer, withdrawal, return to the state, recognitions invalid and destructions of the passport of the citizen of Ukraine for the trip abroad approved by the resolution of the Cabinet of Ministers of Ukraine of May 7, 2014 No. 152 (The Official Bulletin of Ukraine, 2014, No. 45, Art. 1188; 2017, No. 2, Art. 38; 2022, No. 87, the Art. 5397), and in the Procedure for registration, issue, exchange, transfer, withdrawal, return to recognition invalid and to destruction of the passport of the citizen of Ukraine approved by the resolution of the Cabinet of Ministers of Ukraine of March 25, 2015 No. 302 (The Official Bulletin of Ukraine, 2015, No. 40, Art. 1188; 2016, No. 87, Art. 2829; 2021, No. 86, the Art. 5479), the enclosed changes.

2. Determine that before introduction of interdepartmental information exchange between the Unified state demographic register and the State register of acts of civil status of citizens, Ukraine provided by the Laws "About the Unified state demographic register and documents confirming citizenship of Ukraine, proving the identity or her special status" and "About state registration of acts of civil status" in case of execution of the passport of the citizen of Ukraine, the passport of the citizen of Ukraine for trip abroad of the data on the birth of physical person and its origin, adoption, deprivation and recovery of the parent rights, marriage, annulment of marriage, change of name, death are acquired by the Public migratory service, its territorial bodies / territorial subdivisions, foreign diplomatic organizations in the form of extraction by direct access (in case of availability of technical capability) in the State register of acts of civil status of citizens. the specified extraction about state registration of the birth is entered into departmental information system of the Public migratory service / departmental information system of the Ministry of Foreign Affairs and applied to the statement questionnaire.

Changes which are made to the procedures approved by resolutions of the Cabinet of Ministers of Ukraine of May 7, 2014 No. 152 and of March 25, 2015 No. 302

1. According to the procedure of registration, issue, exchange, transfer, withdrawal, return to the state, recognitions invalid and destructions of the passport of the citizen of Ukraine for trip abroad, No. 152 approved by the resolution of the Cabinet of Ministers of Ukraine of May 7, 2014:

Item 14 to add 1) with the paragraph of the following content:

"In case when the certificate of birth of person who was born in the territory of Ukraine or the statement from the State register of acts of civil status of citizens about state registration of the birth does not move the applicant for execution of the passport for trip abroad, in the statement questionnaire the corresponding mark becomes.";

Item 17 to state 2) in the following edition:

"17. In case of absence in the Register of data on the birth of physical person and its origin, adoption, deprivation and recovery of the parent rights, marriage, annulment of marriage, change of name, death which is subject to obligatory entering into the Register in the amount necessary for identification of person in connection with registration (including instead of lost or stolen), exchange of the passport of the citizen of Ukraine for trip abroad according to the Law of Ukraine "About the Unified state demographic register and documents confirming citizenship of Ukraine, proving the identity or her special status" such data from the State register of acts of civil status of citizens turn out according to the procedure of interdepartmental electronic information exchange of the Register and the State register of acts of civil status of citizens through system of electronic interaction of the state electronic information resources according to the procedure, stipulated by the legislation.

If information on person in the State register of acts of civil status of citizens is absent or information on person requires check, receipt of information about person is performed by the direction of requests in departments of state registration of acts of civil status of citizens and provision of answers to them in paper form.";
