In compliance with the subitem 176) of Item 15 of the Order of the Government of the Republic of Kazakhstan of February 18, 2017 I ORDER to No. 81 "Some questions of the Ministry of Labour and Social Protection of the population of the Republic of Kazakhstan":

1. Approve document forms, created when conducting medico-social examination:

1) register of certificates of disability and extent of loss of professional working capacity according to appendix 1 to this order;

2) the magazine of registration of issue of the certificate of disability according to appendix 2 to this order;

3) the magazine of registration of issue of the certificate of extent of loss of general working capacity according to appendix 3 to this order;

4) the magazine of registration of issue of the certificate of extent of loss of professional working capacity and the statement from the certificate of extent of loss of professional working capacity and the act of medico-social examination according to appendix 4 to this order;

5) the magazine of registration of issue of the conclusions about needs of the injured worker in additional types of the help and leaving according to appendix 5 to this order;

6) register of movement of acts of medico-social examination according to appendix 6 to this order;

2. Recognize invalid some orders and structural element of the order of the Ministry of Labour and Social Protection of the population of the Republic of Kazakhstan according to appendix 7 to this order.

3. To provide to social services department of the Ministry of Labour and Social Protection of the population of the Republic of Kazakhstan in the procedure established by the legislation:

1) state registration of this order in the Ministry of Justice of the Republic of Kazakhstan;
