Bring in Regulations on procedure, conditions and criteria of the establishment of disability approved by the resolution of the Cabinet of Ministers of Ukraine of December 3, 2009 No. 1317 "Questions of medico-social examination" (The Official Bulletin of Ukraine, 2009, No. 95, Art. 3265; 2012, No. 42, Art. 1618; 2022, No. 28, the Art. 1550), the enclosed changes.

1. State item 4 in the following edition:
"4. The medical and advisory commission of healthcare institution directs persons addressing for disability establishment, to survey of the commission according to this Provision.
For passing of medico-social examination person addressing for disability establishment shall file the following documents:
the passport of the citizen of Ukraine and the statement from the register of territorial community (in case of presentation of the passport of the citizen of Ukraine in the form of plastic card (ID card), the certificate of capture on accounting of the internally displaced person - for the internally displaced person (in the presence);
