About approval of types of certificates on state registration of acts of civil status

1) types of certificates on state registration of acts of civil status in digital format, held for use in the territory of the Kyrgyz Republic, according to appendix 1;

2) blank forms of certificates on state registration of acts of civil status, held for use in the territory of the Kyrgyz Republic and beyond its limits, according to appendix 2.

Determine that issue of forms of certificates of the civil registrations made according to the order of the Government of the Kyrgyz Republic "About approval of blank forms of statements for state registration of acts of civil status, blank forms of civil registrations and procedure for their filling, forms (the description and samples) of forms of certificates on state registration of acts of civil status and procedure for their filling and blank forms of the documents confirming the facts of state registration of acts of civil status" of December 11, 2018 No. 579, is performed before their termination.
