Approve application forms about provision of licenses, statements for the termination of implementation of the licensed type of activity, statements for renewal of the document confirming license availability (prolongation of effective period of the license), notifications on refusal in renewal of the document confirming license availability (prolongation of effective period of the license) (the appendices NN1, 2, 3, 4).

3. To impose control of execution of this order on Shestopalov N. V.
