"19. To parents or guardians of children with disability:

1) the passport copy or other identity document of person (including in digital format by means of the Tunduk mobile application);

2) the document confirming the status of the guardian;

3) copy of the certificate of birth of the child;

4) copy of the conclusion of medico-social commission of experts about establishment of group of disability of the child.".
