About approval of the standard form of the payment notice on payment of the single tax of independent entrepreneurs

For the purpose of execution of provisions of Chapter 10-4 of the Section II of the Tax code (Monitorul Oficial al Republicii Moldova of February 8 2007, special release) I ORDER:

1. Approve the standard form "The Payment Notice on Payment on the Single Tax of Independent Entrepreneurs", according to appendix No. 1.

2. Approve the Procedure for filling of the payment notice on payment of the single tax of independent entrepreneurs, according to appendix No. 2.

3. Approve the standard form "The Statement for Correction of the Income Amount Reflected in the Payment Notice on Payment on the Single Tax, Independent Entrepreneurs", according to appendix No. 3.

4. Approve Regulations on correction of the payment notice on payment of the single tax of independent entrepreneurs.

5. This order becomes effective from the date of publication in Monitorul Oficial al Republicii Moldova.
