1. Approve enclosed:

1) the list of the reporting under the arrived appeals of physical persons and legal entities (further – consumers of financial services) banks, branches of banks – nonresidents of the Republic of Kazakhstan, the organizations performing separate types of banking activities, insurance companies, branches of insurance companies – nonresidents of the Republic of Kazakhstan, the organizations performing broker activities in the security market, the microfinancial organizations (further – the financial organizations);

2) report form about addresses of consumers of financial services (aggregative data);

3) report form about financial effects for consumers of financial services in results of consideration of addresses;

4) Rules of submission by the financial organizations of the reporting under the arrived addresses of consumers of financial services.

2. The financial organizations represent to Agency of the Republic of Kazakhstan on regulation and development of the financial market:

1) the report on the form provided by the subitem 2) of Item 1 of this resolution – once a half-year, till 15 (fifteenth) (inclusive) the second month following the accounting period;

2) the report on the form provided by the subitem 3) of Item 1 of this resolution – once a half-year, till 15 (fifteenth) (inclusive) the second month following the accounting period.

3. To provide to department of behavioural supervision in the procedure established by the legislation of the Republic of Kazakhstan:

1) together with Legal department state registration of this resolution in the Ministry of Justice of the Republic of Kazakhstan;

2) placement of this resolution on official Internet resource of the Agency of the Republic of Kazakhstan on regulation and development of the financial market after its official publication;

3) within ten working days after state registration of this resolution submission to Legal department of data on execution of the action provided by the subitem 2) of this Item.

4. To impose control of execution of this resolution on the supervising vice-chairman of the Agency of the Republic of Kazakhstan on regulation and development of the financial market.

5. This resolution becomes effective after sixty calendar days after day of its first official publication.
