Based on part three of Item 6 of article 85 of the Tax Code of the Republic of Belarus, the paragraph of third No. 201 "About submission of data on the income of physical persons" the Ministry of Taxes and Tax Collection of the Republic of Belarus DECIDES: parts two of Item 2 of the resolution of Council of Ministers of the Republic of Belarus of April 7, 2021
1. Establish forms of data:
about the income (payments) of the physical persons represented by the Ministry of Labour and Social Protection according to appendix 1;
about the income (payments) of the physical persons represented by the Ministry of Finance according to appendix 2;
about payments (income) of physical persons on which compulsory insurance premiums, payments at the expense of the means of the national social insurance represented by Social Security Fund of the population of the Ministry of Labour and Social Protection according to appendix 3 are assessed;
about pensions of physical persons from other states represented by Social Security Fund of the population of the Ministry of Labour and Social Protection according to appendix 4;
about professional pensions of the physical persons represented by Social Security Fund of the population of the Ministry of Labour and Social Protection according to appendix 5;
about income of physical persons gained as a result of alienation of real estate, except for the companies as property complexes and also the list of the data necessary for implementation of tax control concerning the real estate units represented by the Research and production state republican unitary enterprise "National Cadastral Agency" according to appendix 6;
about the income (payments) of the physical persons represented by the Belarusian republican unitary insurance enterprise Belgosstrakh according to appendix 7;
about the issued and (or) listed money which arrived to physical persons transfer from abroad represented by operators of mail service and banks according to appendix 8;
about the income of the physical persons recognized by the taxation objects the income tax from the physical persons assessed on the different income tax rates from physical persons, including data on privileges and the income tax amounts from the physical persons represented by the organizations 1, by the representative offices of the foreign organizations opened according to the procedure, established by the legislation, the Belarusian individual entrepreneurs, branches 2, and also the branches of the foreign organizations recognized by tax agents according to appendix 9.
