I ORDER:

1. Approve Changes in form of the Tax declaration of ecological tax approved by the order of the Ministry of Finance of Ukraine of August 17, 2015 No. 715, of Ukraine registered in the Ministry of Justice on September 03, 2015 at No. 1052/27497, which are applied.

2. In accordance with the established procedure to provide to department of tax policy of the Ministry of Finance of Ukraine:

submission of this order on state registration in the Ministry of Justice of Ukraine;

promulgation of this order.
