I ORDER:

1. Approve the unified form of the statement which is drawn up by results of holding planned (unplanned) action of the state supervision (control) for observance by the subject of managing of requirements of the legislation in the field of the organization and carrying out gamblings which is applied.

2. Recognize to invalid:

1) the decision of the Commission on regulation of gamblings and lotteries of October 25, 2021 No. 681 "About approval of the unified form of the statement which is drawn up by results of holding planned (unplanned) action of the state supervision (control), concerning observance by the subject of managing of requirements of the legislation in the field of the organization and carrying out gamblings", registered in the Ministry of Justice of Ukraine on November 15, 2021 at No. 1490/37112;
