1. Approve:

N1-OA "Report on Amounts of Acquisition and Realization of Alcoholic Beverages" form;

N1-OT "Report on Amounts of Acquisition and Realization of Tobacco Products" form;

The instruction concerning filling of the N1-OA "Report on Amounts of Acquisition and Realization of Alcoholic Beverages" Form;

The instruction concerning filling of the N1-OT "Report on Amounts of Acquisition and Realization of Tobacco Products" Form which are applied.

2. To submit to department concerning administration of the excise duty and control of production and the address of excise goods this order to the Ministry of Justice of Ukraine for state registration.

3. Declare invalid the order GNA of Ukraine of 21.07.2004 N408 "About Approval of Forms of the Reporting on Purchasing Amounts and/or Realization of Alcoholic Beverages and Tobacco Products" registered in the Ministry of Justice of Ukraine 26.07.2004 for N921/9520 (with changes and amendments).
