According to Item 1 of the resolution of the Cabinet of Ministers of Ukraine of 21.11.2007 No. 1346 "About approval of the Regulations on the certificate of the crew member" I ORDER:

1. Approve the Procedure for issue, storage and destruction of the certificate of the crew member which is applied.

2. Public aviation administration (Lisnyak O. I.):

2.1. Provide provision of this order on state registration in the Ministry of Justice of Ukraine in accordance with the established procedure.

2.2. Bring this order to permission of heads of airlines and airlines of civil aviation of all patterns of ownership.

3. Declare invalid the order of the Ministry of transport of Ukraine of 21.12.94 No. 630 "About approval of the Instruction about procedure for issue, storage and destruction of the certificate of the crew member", registered in the Ministry of Justice of Ukraine 26.12.94 for No. 316/526.

4. This order becomes effective along with the resolution of the Cabinet of Ministers of Ukraine of 21.11.2007 No. 1346 "About approval of the Regulations on the certificate of the crew member".

5. To impose control of execution of this order on the deputy minister - the head of Public aviation administration of O. Davydov.

Procedure for issue, storage and destruction of the certificate of the crew member

I. General provisions

1.1. This Procedure developed according to the resolution of the Cabinet of Ministers of Ukraine of 21.11.2007 No. 1346 "About approval of the Regulations on the certificate of the crew member" determines the procedure of registration, issue, storage and destruction of the certificate of the crew member (further - the certificate) and interaction between State aviaservice of Ukraine and the Ministry of Internal Affairs concerning certification.

1.2. This procedure extends to heads of airlines and airlines, and also persons who are crew members of the aircraft registered in Ukraine according to the Convention on international civil aviation of 1944.

1.3. Registration and certification is performed by State aviaservice of Ukraine means of the State information system of registration accounting of physical persons and their documentation (further - GIS).

1.4. The sample of the certificate of the crew member is given in appendix 1.

1.5. For the purpose of check of reliability of personal data of the receiver of the certificate between State aviaservice of Ukraine and the Ministry of Internal Affairs exchange of information which contains graphical and text data and is transferred by secure channels of communication and/or with use of optical data carriers is performed.

1.6. Subjects of information exchange are:

from State aviaservice of Ukraine - the relevant structural division on activity;

from the Ministry of Internal Affairs - the Main computer center of GIS of State department for nationality, immigrations and registration of physical persons (further - GVTs GIS).

1.7. Component of GIS is the specialized node established in the relevant structural division of State aviaservice of Ukraine (further - the Node).

1.8. State aviaservice of Ukraine and the Ministry of Internal Affairs provide confidentiality of the acquired information and its use is exclusive with the office purpose for execution of the certificate. Such information is not subject to distribution and transfer to the third party.

II. Order of interaction of State aviaservice of Ukraine and the Ministry of Internal Affairs in case of execution of the certificate

2.1. For execution of the certificate the head of airline or airline provides such documents in State aviaservice of Ukraine:

representation (appendix 2);

passport of the citizen of Ukraine;

the certificate of assignment of identification number (card of the physical person - the taxpayer);

the passport of the citizen of Ukraine for trip abroad (in the presence);

the document containing data on blood type;

earlier received certificate (in the presence);

document on payment of the state collection for certification.

2.2. Responsible person of structural division of State aviaservice of Ukraine:

accepts documents, stipulated in Item 2.1 it undressed, checks availability of their complete set and registers them in register of receipt of documents for execution of the certificate of the crew member (appendix 3);

specifies in the list of documents in case certification (appendix 4) the filed documents with indication of the number of their sheets;

in the presence of the receiver of the certificate fills in the questionnaire of the receiver of the certificate of the crew member (appendix 5).

2.3. In the questionnaire the following data on the receiver of the certificate are entered:

a) face:

printing photoprint of face of the receiver of the certificate;

surname, name and middle name;

personal number (identification number on GRFL);

floor;

birth date;

birthplace (state, area, area/city, settlement/village);

residence (postal index, area, area/city, settlement/village, street, house number, case, number of the apartment);

series, number, date of issue of the passport of the citizen of Ukraine and the body which issued it;

place of employment and position (in the Ukrainian and English languages);

number and date of representation, number and date of the order of the head of airline or airline (basis for certification);

series, number, date of issue, effective period of the passport of the citizen of Ukraine for trip abroad and the body which issued it;

blood type and Rhesus factor;

contact telephone number;

certificate effective period;

the surname and name of the receiver of the certificate executed by letters of the Latin alphabet according to rules of transliteration;

date of filling of the questionnaire;

signature of the receiver;

the signature of the head of the structural unit of State aviaservice of Ukraine or person fulfilling its duties.

b) reverse side:

data on payment of the state collection (account number, payment date, amount);

information about the responsible person of State aviaservice of Ukraine who gives personal data of the receiver of the certificate for their check (surname, the signature and date);

number of the drawn-up certificate;

information about the responsible person of State aviaservice of Ukraine who issued the certificate (surname, the signature and date);

data on receipt of the certificate (date and signature of the receiver).

2.4. After filling of the questionnaire the receiver of the certificate with own signature validates the personal data entered in the questionnaire.

2.5. From the personal data of the receiver of the certificate of State aviaservice of Ukraine specified in the questionnaire such data are transferred to GVTs GIS:

the digitized photoprint of face of the receiver of the certificate;

surname, name and middle name;

personal number (identification number on GRFL);

floor;

birth date;

birthplace (state, area, area/city, settlement/village);

residence (postal index, area, area/city, settlement/village, street, house number, case, number of the apartment);

series, number, date of issue of the passport of the citizen of Ukraine and the body which issued it;

series, number, date of issue, effective period of the passport of the citizen of Ukraine for trip abroad and the body which issued it;

the digitized signature of the receiver;

the surname and name of the receiver of the certificate executed by letters of the Latin alphabet according to rules of transliteration.

2.6. GVTs GIS in connection therewith Rather:

performs the personal data of the receiver of the certificate specified in Item 2.5 of this Section;

transfers results of check to State aviaservice of Ukraine.

2.7. By results of verification of documents for receipt of the certificate by the head of the structural unit of State aviaservice of Ukraine responsible for certification, or person fulfilling its duties makes the decision on issue (or refusal in issue) certificates.

2.8. In certification it can be refused case:

the applicant's not belongings to the list of faces of aviation personnel which are crew members;

provisions of the incomplete document package determined in Item 2.1 of this Section;

provisions of unreliable information.

2.9. The applicant can appeal the decision on refusal in certification in the procedure determined by the legislation of Ukraine.

2.10. After registration and certification, and also in case of its loss or withdrawal (return) by State aviaservice of Ukraine such data on the owner of the certificate are transferred to GVTs GIS:

series and number issued (lost, withdrawn/returned) certificates;

surname, name and middle name;

personal number (identification number on GRFL).

2.11. The personal data entered in the questionnaire of the receiver are stored in full in the database of the Node, and regarding the transferred data - in the GVTs GIS database.

Information storage duration - to the special order of the owner of information.

2.12. Subjects of information exchange provide timeliness and provision in full to reliable information and report about the circumstances important for accomplishment of this Procedure.

III. Certification

3.1. Direct registration and certification is performed by State aviaservice of Ukraine in the room which is specially equipped for this purpose.

3.2. The basis for registration and certification is the decision made by the head of the relevant structural division State aviaservice of Ukraine or person fulfilling its duties that is confirmed by its signature in the questionnaire and representation.

3.3. The certificate is issued to the receiver of the certificate not later than in thirty days after filling of the questionnaire. About certification by the responsible person in the questionnaire the corresponding mark becomes, and the fact of its obtaining makes sure the signature of the receiver of the certificate or other person by proxy.

3.4. The certificate is issued to the crew member for a period of up to five years.

3.5. In case of change of place of employment or the termination of flight work the crew member hands over the certificate of airline or to airline where he worked what the head of airline or airline within three working days informs in writing State aviaservice of Ukraine on. The certificate is transferred to State aviaservice of Ukraine no later than ten days from the date of its delivery.

3.6. The certificate which is not handed over in the procedure established by the legislation is considered lost.

3.7. The crew member shall make thrifty use of storage of the certificate and always have it with himself during execution of the international flights.

3.8. In case of loss of the certificate the new certificate according to Sections II and III of this Procedure is issued to its owner.

IV. Accounting of forms of certificates

4.1. Forms of the certificate are subject to accounting as documents of the strict reporting.

4.2. In the relevant structural division of State aviaservice of Ukraine it is recorded forms of certificates:

in register of receipt of forms of the certificate of the crew member (appendix 6);

in the magazine of issue of forms of the certificate of the crew member (appendix 7);

in the magazine of certification of the crew member (appendix 8);

in the magazine of the lost and withdrawn (returned) certificates of the crew member (appendix 9).

4.3. In airlines, airlines in register of certificates of the crew member (appendix 10) is recorded the received, damaged and lost certificates.

4.4. Registers of forms of the certificates issued, lost and the withdrawn (returned) certificates are stored in the procedure established by the legislation of Ukraine.

4.5. In the database of the Node of State aviaservice of Ukraine there is in full information on the certificates of the crew member issued, withdrawn (returned), lost and destroyed.

V. Procedure for storage of the certificate

5.1. Certificates are stored separately in such procedure:

forms of certificates - on sequence numbers;

the withdrawn (returned) certificates - in alphabetical order on surnames of their owners;

the damaged forms of the certificate and the damaged certificates - on first come.

5.2. Forms of the certificate and certificates are stored in metal cases (safes). Metal cases (safes) shall be in specially allocated, isolated, reliably closed room which after completion of work of organization is sealed and surrenders under protection in the procedure established by the legislation.

VI. Procedure for destruction of certificates

6.1. The forms of certificates damaged in case of registration, and the damaged certificates are liable to destruction of State aviaservice of Ukraine by quarterly relevant structural division.

6.2. Destruction of the damaged forms of certificates and the damaged certificates is performed quarterly with execution of the act of destruction which affirms the head of State aviaservice of Ukraine or person fulfilling its duties.
