According to the subitem "ї" subitem 4.4.2 of Item 4.4 of article 4 of the Law of Ukraine "About procedure for repayment of obligations of taxpayers to budgets and the state trust funds" I ORDER:

1. Approve form of tax calculation of collection for pollution of the surrounding environment which is applied.

2. Approve the Instruction concerning creation of tax calculation of collection for pollution of the surrounding environment which is applied.

3. To department of methodology (Flissak N. P.):

submit this order on approval to Committee of the Verkhovna Rada of Ukraine concerning finance and banking activity;

after approval of Committee of the Verkhovna Rada of Ukraine concerning finance and banking activity to submit this order to the Ministry of Justice of Ukraine on state registration.

4. To department of servicing of taxpayers (Kosarchuk V. P.) and to Management of information policy (Maister A. V.) in 10-day time from the date of registration of this order in the Ministry of Justice of Ukraine to provide its placement on the website of GNA of Ukraine and respectively through mass media to bring to permission of taxpayers.

5. To impose control of execution of this order on the vice-chairman of GNA of Ukraine Doctor S. I.
