About approval of the Procedure for declaring of production volumes and turnover of ethyl alcohol, alcoholic and alcohol-containing products

1. Approve:

- Procedure for declaring of production volumes and turnover of ethyl alcohol, alcoholic and alcohol-containing products according to appendix 1;

- form of the Declaration on production volumes and turnover of alcoholic products (FORM STI-144) according to appendix 2;

- the Declaration form of actually made and realized amounts of ethyl alcohol" (FORM STI-145) according to appendix 3;

- form of the Declaration on the predicted production volumes of ethyl alcohol and alcoholic products the forthcoming accounting year (FORM STI-146) according to appendix 4;

- the Declaration form of actually imported and realized amounts of ethyl alcohol and alcoholic products (FORM STI-147) according to appendix 5;

- form of the Declaration on amounts of the realized ethyl alcohol on the companies (FORM STI-148) according to appendix 6;

- form of the Declaration on amounts of the used ethyl alcohol special consumers (FORM STI-149) according to appendix 7.
