Approve:

report form about use of the amounts of insurance premiums for compulsory social insurance from labor accidents and occupational diseases on financing of preventive measures on reducing industrial traumatism and occupational diseases of workers (appendix 1);

procedure for submission of the report on use of the amounts of insurance premiums for compulsory social insurance from labor accidents and occupational diseases on financing of preventive measures on reducing industrial traumatism and occupational diseases of workers (appendix 2).

Appendix 1

The report form is constituted by the insurer the accruing result since the beginning of year and it is represented in executive bodies of Social Insurance Fund of the Russian Federation in place of registration along with the pay-sheet on fund of social insurance of the Russian Federation (form 4-FSS RF)
