According to articles 18 and 39 of the Law of Ukraine "About telecommunications" the National commission on questions of regulation of communication of Ukraine SOLVED:

1. Approve forms of the reporting of operators of telecommunications (further - forms) which are applied:

1.1. N 1-FM "The activities report of the operator of the local fixed telephone communication".

1.2. N 2-FG "The activities report of the operator of the long distance fixed telephone communication".

1.3. N 3-FN "The activities report of the operator of the international fixed telephone communication".

1.4. N 4-MZ "Activities report of mobile operator".

1.5. N 5-KT "Activities report of the operator of telecommunication networks of cable television broadcasting".

2. Approve instructions concerning filling of the forms of the reporting approved by Item 1 of this decision (further - instructions) which are applied:

2.1. The instruction concerning filling of form of the reporting N 1-FM "The activities report of the operator of the local fixed telephone communication".

2.2. The instruction concerning filling of form of the reporting N 2-FG "The activities report of the operator of the long distance fixed telephone communication".
