About approval of the Procedure for provision of data on the main results of order fulfillment on carrying out research and development of project and design works, actions for ensuring their practical application and about results of monitoring of implementation of scientific (scientific and technical) products
According to Item 15 of the Procedure for forming and order fulfillment on carrying out research and development, project and design works at the expense of means of the government budget approved by the resolution of the Cabinet of Ministers of Ukraine of 25.08.2004 N 1084 (with changes), and for the purpose of strengthening of responsibility of customers - the main managers of budgetary funds for implementation of results of the order I ORDER:
1. Approve the Procedure for provision of data on the main results of order fulfillment on carrying out research and development, project and design works, actions for ensuring their practical application and about results of monitoring of implementation of scientific (scientific and technical) products which is applied.
2. To department of scientific technology development (Svizhenko I. O.):
2.1. Provide the organization of preparation and submission to the Cabinet of Ministers of Ukraine of the annual research opinion about condition of development of science and technology and results of scientific and scientific and technical activities according to this Procedure.
2.2. Promulgate this Procedure on the official site of MON of Ukraine.
2.3. Submit this order on state registration to the Ministry of Justice of Ukraine.
3. To impose control over the implementation of this order on the deputy minister of science and education of Ukraine to Strikh M. V.

1.1. This Procedure is developed according to Item 15 of the Procedure for forming and order fulfillment on carrying out research and development, project and design works at the expense of means of the government budget approved by the resolution of the Cabinet of Ministers of Ukraine of 25.08.2004 N 1084 (with changes).
1.2. The procedure is directed to strengthening of responsibility of customers for implementation of results of the order, providing continuity of accomplishment of stages of scientific process (basic researches, applied researches and scientific and technical developments) and control of bringing results of the order to stage of their practical application, and also determines the main requirements to the organization of preparation, structure and form of submission by customers of data on the main results of order fulfillment, action for ensuring their practical application and about results of monitoring of implementation of scientific (scientific and technical) products (further - NTP) which is performed within three years from the moment of its creation (further - data).
2.1. For the organization of preparation and submission of data on the main results of order fulfillment customers provide:
provision of information on financing of the order, results of its accomplishment and implementation on forms according to appendices 1-7;
preparation of analytical note according to appendix 8 "Analytical note about the main results of order fulfillment and other directions of financial support of the sphere of scientific and scientific and technical activities" by results of generalization and information analysis;
submission of data to the Ministry of Education and Science on paper and electronic media in the established forms not later than March 15 of the year coming behind reporting.
2.2. The Ministry of Education and Science generalizes the data submitted customers, carries out the analysis of effectiveness of order fulfillment, submits to the Cabinet of Ministers of Ukraine the research opinion about condition of development of science and technology and results of scientific and scientific and technical activities not later than June 15 of the year coming for reporting.
3.1. In the N 1 form "Dynamics of financing of the sphere of scientific and scientific and technical activities by the directions of budget financing" (further - form N 1) data on planned and actual expenses of the government budget in reporting year (taking into account changes are provided), planned targets next year by budget programs of the customer and the directions of budget financing.
3.2. In the N 2 form "Budget financing of the scientific sphere by basic and program and target methods, work types and sectors of science" (further - form N 2) data on the actual amounts basic and program target financing (expense amount of general and special funds of the government budget) in the directions of budget financing, sectors of science and types of scientific and scientific and technical works are provided.
3.3. In the N 3 form "Specification on condition of accomplishment and financing of projects according to the state target scientific and scientific and technical programs and scientific parts of the state target programs" (further - form N 3) data on condition of accomplishment and financing of works (projects) on the state target scientific and scientific and technical programs and scientific parts of the state target programs (GTsNTP) by GTsNTP and budget programs are provided.
3.4. In the N 4 form "The main results of order fulfillment" (daleeforma N 4) indicators of effectiveness of order fulfillment in reporting year by the directions of budget financing on general and special funds are given.
3.5. In the N 5 form "Data on the most powerful results of order fulfillment" (further - form N 5) data on the most powerful results of order fulfillment are provided.
