According to the subitem 8 of Item 10 of the Regulations on purchase of goods, works and services for public funds approved by the resolution of the Cabinet of Ministers of Ukraine of 17.10.2008 N 921 (with changes), I ORDER:

1. Approve:

form of the announcement of carrying out the open and two-level tenderings and the Instruction concerning its filling;

form of the announcement of the results of the procedure of purchase and the Instruction concerning its filling;

form of the announcement of carrying out preliminary qualification and the Instruction concerning its filling;

report form about results of holding procedures of the open and two-level tenderings and the Instruction concerning its filling;

report form about the results of the procedure of the biddings with limited participation and Instructions concerning its filling;

report form about the results of the procedure of request of price offers (quotations) and the Instruction concerning its filling;

report form about the results of the procedure of purchase at one participant and the Instruction concerning its filling;

form of the protocol on disclosure of tender (price) offers;

form of the protocol of assessment of tender (price) offers;

form of the register of the received tender (price) offers;

form of the annual plan of purchases and the Instruction concerning its filling (are applied).

2. Repeal the order of Ministry of Economics of 05.11.2008 N 631 "About approval of document forms in the field of public procurements".
