1. Establish:
the application form about state registration of fund according to appendix 1;
the application form about state registration of the changes and (or) additions made to the fund charter according to appendix 2;
form of the questionnaire of the founder of fund according to appendix 3;
form of the certificate on state registration of fund according to appendix 4.
2. Determine that the form of the certificate on state registration of fund is made in the A4 format.
2-1. Determine that funds annually till March 1 bring to the general attention by placement in mass media or on the official sites on the global computer Internet (in case of their availability) and (or) by the direction in the place of state registration in the Ministry of Justice, head departments of justice regional, Minsk city executive committees (further - judicial authority) for the subsequent placement on the official site of relevant organ of justice on the global computer Internet information (reporting) about:
the activities;
receipt and expenditure of money and other property.

continuation of activities of fund with indication of the location of governing body and documents confirming location availability;
