About approval of forms of the notice and statements which are drawn up in case of the item-by-item termination (restriction) of gas supply to consumers, except the population

According to the resolution of the Cabinet of Ministers of Ukraine of 08.12.2006 N 1687 "About approval of the Procedure for the item-by-item termination (restriction) of gas supply to consumers, except the population" I ORDER:

1. Approve document forms on the termination (restriction) of gas supply:

1.1. The notice on the termination (restriction) of gas supply which is attached.

1.2. The act of the termination (restriction) of gas supply and sealing of entrance sliding armature of the gas pipeline or gas distribution point (in case of its availability) which is applied.

1.3. The act on removal of seal and recovery of gas supply which is attached.

2. To provide to the director of the department of questions of oil, gas and oil-processing industry (Nesterenko O. G.) provision of this order on state registration in the Ministry of Justice of Ukraine in accordance with the established procedure.

3. To impose control over the implementation of this order on the deputy minister Burzu Aliyev.

4. This order becomes effective in 10 days after its state registration in the Ministry of Justice of Ukraine.
