About approval of forms of certificates of accreditation

For the purpose of implementation of the order of the Government of the Russian Federation of February 24, 2009 N 163 "About accreditation of certification bodies and test laboratories (centers) performing works on confirmation of conformity" (The Russian Federation Code, 2009, N 10, to the Art. 1221), I order:

1. Approve:

forms of certificates of accreditation of certification body and the test laboratory (center) performing works on confirmation of conformity and appendices to them (are applied);

recommendations about filling of form of certificates of accreditation of certification body and the test laboratory (center) performing works on confirmation of conformity (are attached).

2. Determine that:

forms of certificates of accreditation of certification body and test laboratory (center) are the printed material of level of protection protected from counterfeits "In", are made in accordance with the established procedure and are subject to strict accounting;

registration number of certificates of accreditation of certification body and test laboratory (center) is created by accreditation body according to statutory rules;

the area of accreditation of certification body and test laboratory (center) is specified in appendix to the certificate of accreditation which is integral part of the certificate of accreditation;

appendix to the certificate of accreditation is drawn up on sheets A4. Sheets of appendix to the certificate of accreditation are numbered, sewed and sealed accrediting body in accordance with the established procedure.

Recommendations about filling of form of certificates of accreditation of certification body and the test laboratory (center) performing works on confirmation of conformity

1. In the line "name of accreditation body" full name of accreditation body is specified.

2. In the line "N" number of the certificate of accreditation of certification body and test laboratory (center) which structure is created by accreditation body according to statutory rules is specified.

3. In the line "this certificate is issued" data of the applicant applying for accreditation as certification body or test laboratory (center) are specified:

for the legal entity - full name, including form of business, the primary state registration number of record about state registration of the legal entity (further - PSRN), the address (location) according to the document confirming the fact of entering of record about state registration into the Unified State Register of Legal Entities;

for the individual entrepreneur - surname, name, middle name, the address (residence) according to the document confirming the fact of entering of record about state registration into the Unified State Register of Private Entrepreneurs, the primary state registration number of record about state registration of the individual entrepreneur (further - PSRNSP).

4. In the line "also certifies that" data of certification body or test laboratory (center) are specified: full name, address (location).

5. In the line "conforms to requirements" the regulatory legal acts and/or regulating documents containing requirements to certification bodies and test laboratories (centers) performing activities in the field of confirmation of conformity are specified.

6. In the certificate of accreditation of test laboratory (center) in the line "it is accredited on" it is specified:

for the test laboratory (center) which is part of the legal entity answering to the principle of technical regulation about independence of manufacturers, sellers, contractors and acquirers - that the specified laboratory (center) is accredited on technical competence and independence;

for the test laboratory (center) which is part of the legal entity who is not answering to the principle of technical regulation about independence of manufacturers, sellers, contractors and acquirers - that the specified laboratory (center) is accredited on technical competence.

7. In the line "effective period of the certificate of accreditation" start date of effective period and expiry date of the certificate of accreditation of certification body and test laboratory (center) is specified.

Start date of effective period of the certificate of accreditation is date of issue of the certificate of accreditation to the applicant which is determined according to Item 17 of the Regulations on accreditation of certification bodies and test laboratories (centers) performing works on confirmation of conformity, approved by the order of the Government of the Russian Federation of February 24, 2009 N 163.

c) if the area of accreditation of body for certification / test laboratory (center) includes two and more groups of products (services), each group of products (services) is drawn up in appendix by the separate Section;

d) in case of expansion of area of accreditation of certification body or test laboratory (center) the additional area of their accreditation is drawn up by additional appendix to the certificate of accreditation of the specified certification body or test laboratory (center).

In case of registration of additional appendix in the line "Appendix to the Certificate of Accreditation of Body on Certification/appendix to the Certificate of Accreditation of Test Laboratory (Center)" after the word "Appendix" number of this appendix (since N 2 and further) is specified by any printing method taking into account that the appendix establishing the main area of accreditation of accreditation body and test laboratory (center) is considered the first appendix to the certificate of accreditation.
