For the purpose of enhancement of activities of bodies of civil registration, improvement of quality of their work and based on the Regulations on the Ministry of Justice of the Republic of Uzbekistan approved by the resolution of the Cabinet of Ministers of the Republic of Uzbekistan of August 27, 2003 N 370 (Collection of the legislation of the Republic of Uzbekistan, 2003, N 15-16, Art. 138), order:

1. Approve the following:

"Form of the birth statement" according to the appendix N 1 (the text is provided in Uzbek);

"Form of record of the act of marriage" according to the appendix N 2 (the text is provided in Uzbek);

"Form of record of the act of annulment of marriage" according to the appendix N 3 (the text is provided in Uzbek);

"Rules of filling of civil registrations" according to the appendix N 5 (the text is provided in Uzbek).

2. Make state registration of this order and enter the corresponding record in the State register of normative legal acts of the ministries, state committees and departments of the Republic of Uzbekistan.

3. To management of systematization of the legislation and legal information (D. Sajdullayev) and to Management of notariate, the REGISTRY OFFICE and legal professions (D.
