1. Approve the Procedure for issue of the certificate of the income tax paid by the nonresident in Ukraine (income) (further - the Procedure) which is applied.

2. Approve reference form of the income tax paid by the nonresident in Ukraine (income) which is applied.

3. To the head of department of international backgrounds Gladun Yu. V.:

3.1. Submit this order to the Ministry of Justice of Ukraine for state registration.

3.2. After state registration of this order to send reference form of the income tax paid by the nonresident in Ukraine (income) to competent authorities of each of the states with which Ukraine signed the international agreements on avoidance of double taxation.

4. The order becomes effective with entry into force of the Law of Ukraine "About tax on the income of physical persons".

5. To the director of the department of public relations and mass and explanatory work Kosarchuk V. P. and editor-in-chief of the "Bulletin of Tax Service of Ukraine" magazine Kudritsky D. A. prove this order through mass media to permission of taxpayers.
