About approval of forms of tax declarations of collection for parking lots of vehicles and tourist tax

1. Approve forms:

1.1 the tax declaration of collection for parking lots of the vehicle which is attached;

1.2 the tax declaration of tourist tax which is attached.

2. Recognize invalid orders of State Tax Administration of Ukraine:

of 24.12.2003 N 621 "About approval of form of tax calculation of collection for prize on races on hippodrome", registered in the Ministry of Justice of Ukraine 09.01.2004 for N 9/8608;
