1. Approve form of the tax declaration of collection for parking lots of vehicles which is applied.

2. Approve form of the tax declaration of tourist tax which is applied.

3. Declare invalid the order of State Tax Administration of Ukraine of December 24, 2010 No. 1014 "About approval of forms of tax declarations of collection for parking lots of vehicles and tourist tax", registered in the Ministry of Justice of Ukraine on January 12, 2011 for No. 35/18773.

4. To department of tax, customs policy and methodology of financial accounting of the Ministry of Finance of Ukraine (Chmeruk N. A.), to Department of the taxation of legal entities of the State Tax Administration of Ukraine (Pavelko L. G.) to provide in accordance with the established procedure:

submission of this order in accordance with the established procedure on state registration in the Ministry of Justice of Ukraine;

promulgation of this order.
