About approval of forms and the Procedure for filling of trade patents, commitment forms on acquisition of the trade patent

According to subitems 267.4.1 and 267.4.9 of Item 267.4 of Article 267 of the Section XII of the Tax code of Ukraine, being guided by article 8 of the Law of Ukraine "About the State Tax Service of Ukraine", I ORDER:

1. Approve forms which are applied:

1.1. The trade patent (short-term) for carrying out trading activity;

1.2. The trade patent (preferential) for carrying out trading activity;

1.3. The trade patent for carrying out trading activity, activities for provision of paid household services, trade in currency values, activities in the field of entertainments;

1.4. Requests for acquisition of the trade patent.

2. Approve the Procedure for filling of the trade patent which is applied.

3. To department of administration of the income tax and other taxes and fees (obligatory payments) (Hotsyanivskaya N. V.) in accordance with the established procedure:

3.1 submit this order to the Ministry of Justice of Ukraine for state registration;

3.2 provide promulgation of this order.

4. To department of economic providing (Tsoklan I. I.) organize the timely order of forms of trade patents of new sample and provide with them subordinate bodies of the State Tax Service.
