About approval of sample of the form of the certificate of conformity

On accomplishment of article 11 of the Law of Ukraine "About confirmation of conformity" and article 29 of the Law of Ukraine "About standards, technical regulations and assessment procedures of compliance" I ORDER:

1. Approve sample of the form of the certificate of conformity it (is applied).

2. To the appointed conformity assessment bodies:

take as basis the sample of the form of the certificate of conformity approved by this order during execution of certificates by results of certification in legislatively regulated sphere;

provide maintaining the register of certificates of conformity of the established sample;

provide transfer of copies of certificates of conformity of the established sample in the state register of certificates of conformity.

3. To provide to management of technical regulation (Gordiyenko V. R.) submission of this order on state registration in the Ministry of Justice of Ukraine in the procedure established by the legislation.

4. This order becomes effective in 30 days after day of its official publication.

5. To the state company "Ukrainian Research and Training Center of Problems of Standardization, Certification and Quality" (Kononenko O. V.) to provide the publication of this order in the next edition of the monthly information index "Standards".

6. I reserve control over the implementation of this order.
