1) form of checking leaf of activities of notaries public, according to appendix 1 to this order;

2) form of checking leaf of activities of officials of offices of akims of the cities of district value, settlements, auls (villages), the aulny (rural) districts authorized on making of notarial actions according to appendix 2 to this order;

3) form of checking leaf of activities of the notaries who are engaged in private practice (private notaries) according to appendix 3 to this order;

4) criteria of degree of risks in the field of notarial activities, according to appendix 4 to this order;

5) form of checking leaf of activities of territorial notarial chambers, according to appendix 5 to this order.
