1. Approve enclosed:

1) Criteria for evaluation of degree of risks for the objects which are not relating to the sphere of private entrepreneurship in the field of fire and industrial safety, Civil defense according to appendix 1 to this order;

2) form of checking leaf in the field of fire safety according to appendix 2 to this order;

3) form of checking leaf in the field of industrial safety according to appendix 3 to this order;

4) form of checking leaf in the field of Civil defense according to appendix 4 to this order.

2. To committees of fire service (Aubakirov S. G.), on the state control of emergency situations and industrial safety (Akhmetov S. B). To provide the Ministry of Emergency Situations of the Republic of Kazakhstan:

1) state registration of this order in the Ministry of Justice of the Republic of Kazakhstan;

2) official publication of this order after state registration;

3) publication of this order on the Internet resource of the Ministry of Emergency Situations of the Republic of Kazakhstan.

3. I reserve control of execution of this order.

4. This order becomes effective after ten calendar days after day of its first official publication.
