the application form about liquidation of the permanent reference tribunal created as non-profit organization according to appendix 3;

the application form about registration of the permanent reference tribunal created as separate division (division) of the legal entity according to appendix 4;

the application form about removal from accounting of the permanent reference tribunal created as separate division (division) of the legal entity according to appendix 5;

form of register of the permanent reference tribunals created as separate divisions (divisions) of legal entities according to appendix 6.

2. Approve the Instruction about procedure for maintaining the Register of arbitration judges and permanent reference tribunals it (is applied).
