1. Approve form of the Tax declaration on the income tax of the insurer which is applied.

2. To department of tax, customs policy and methodology of financial accounting of the Ministry of Finance of Ukraine (Chmeruk M. O.), to Department of the taxation of legal entities of the State Tax Administration of Ukraine (Lyubchenko O. M.) in accordance with the established procedure to provide:

2.1. Submission of this order on state registration in the Ministry of Justice of Ukraine.

2.2. Promulgation of this order.

3. To the State Tax Service of Ukraine (Zakharchenko V. Yu.):

3.1. In three-months time of day of state registration of this order in the Ministry of Justice of Ukraine to provide development of algorithms of control of acceptance and the automated processing of the Tax declaration on the income tax of the insurer.

3.2. Finish the software on acceptance and processing of tax statements of taxpayers regarding data processing of the Tax declaration on the income tax of the insurer according to the procedure and the terms determined for creation of information systems.

4. This order becomes effective from the date of official publication.

5. To impose control over the implementation of this order on the First Deputy Minister Myarkovsky A. I.
