2. Declare invalid the resolution of the Cabinet of Ministers of Ukraine of October 8, 1997 No. 1125 "About approval of sample of the form of the questionnaire for registration to foreigners and stateless persons of visas to entry into Ukraine" (The Official Bulletin of Ukraine, 1997, number 41, page 54).

Changes which are made to Rules of execution of visas to entry into Ukraine and transit through its territory

1. In Item 2:

1) paragraphs two, the eighth and tenth to state in such edition:
