1. Approve the Procedure for creation and representation of requests for receipt of public information which manager is the Ministry of Finance of Ukraine (daleeporyadok) which is applied.

2. Approve the Form for representation of request for receipt of public information in writing which is attached.

3. Approve the Form for representation of request for receipt of public information on e-mail which is attached.

4. (Press service) (Kosarchuk V. P.) to provide to management of communications:

placement of the Procedure, Form for representation of request for receipt of public information in writing and Forms for representation of request for receipt of public information on e-mail on the official website of the Ministry of Finance of Ukraine;

submission of this order in accordance with the established procedure on state registration in the Ministry of Justice of Ukraine.

5. This order becomes effective from the date of its official publication.
