According to Item 179.9 of Article 179 of the Section IV of the Tax code of Ukraine I ORDER:

1. Approve form of the tax declaration of property condition and the income and the Instruction concerning filling of the tax declaration about property condition and the income which are attached.

2. Recognize invalid orders of the Ministry of Finance of Ukraine according to the list which is attached.

3. To department of tax, customs policy and methodology of financial accounting (Chmeruk M. O.) in the procedure established by the legislation:

provide this order to the Ministry of Justice of Ukraine on state registration;

provide promulgation of this order.

4. This order becomes effective since January 1, 2012.

5. To impose control over the implementation of this order on the First Deputy Minister Myarkovsky A. I.
