According to the Law of Ukraine "About access to public information", the Presidential decree of Ukraine of 05.05.2011 No. 547 "Questions of ensuring with executive bodies of access to public information", the resolution of the Cabinet of Ministers of Ukraine of 25.05.2011 No. 583 "Questions of accomplishment of the Law of Ukraine "On access to public information" in the Secretariat of the Cabinet of Ministers of Ukraine, central and local executive bodies" I ORDER:

1. Approve the Procedure for creation and representation of requests for public information which manager is the Public archive agency of Ukraine which is applied.

2. Approve request form on information which is attached.

3. (Zerkal O. V.) to submit to department of interaction with authorities this order on state registration according to the Presidential decree of Ukraine of 03.10.92 No. 493 "About state registration of regulatory legal acts of the ministries and other executive bodies" (with changes).

4. To impose control over the implementation of this order on the director of the department of interaction with authorities Zerkal O. V.

5. This order becomes effective from the date of its official publication.
