According to the subitem 4 of item 4 of the Regulations on Public service concerning disabled people and veterans of Ukraine approved by the Presidential decree of Ukraine of 06.04.2011 No. 397, and in pursuance of the Presidential decree of Ukraine of 09.12.2010 No. 1085 "About optimization of system of the central executive bodies" I ORDER:

1. Approve reporting No. 6 form "The report on providing disabled people with special motor transport" and the Instruction concerning filling of form of the reporting No. 6 "The report on providing disabled people with special motor transport" who are applied.

2. Declare invalid the order of the Ministry of Labour and Social Policy of Ukraine of 02.10.2009 No. 372 "About approval of form of the reporting No. 6 "The report on providing disabled people with special motor transport" and the Instruction concerning its filling", registered in the Ministry of Justice of Ukraine 10.12.2009 for No. 1181/17197.

3. To the ministry of social policy of the Autonomous Republic of Crimea, head departments of work and social protection of the population of regional public administrations and the Kiev city public administration, Management of work and social protection of the population of the Sevastopol city public administration to provide provision of the reporting to Public service concerning disabled people and veterans of Ukraine and to head departments of statistics in the Autonomous Republic of Crimea, areas, the city of Kiev and to Management of statistics in the city of Sevastopol annually at the scheduled time.
