1. Make to the Section 5 "Licenses, Permissions, Certificates and Other Documents Which Are Issued by State Bodies of Ukraine" of the Qualifier of documents approved by the order of State Customs Service of 15.02.2011 No. 107 (with changes) such changes:

by the 5038 code to exclude code and the name of the document;

add the Section after code and the name of the document by the 5122 code with new line of such content:
