1.3. Form for representation of request for receipt of public information by legal entities and associations of citizens without the status of the legal entity which is attached.
2. To legal management (Dudchenko V. G.) and Management of documentary providing (Bordenyuk L. L.) submit this resolution on state registration to the Ministry of Justice of Ukraine in accordance with the established procedure.
