About approval of form of primary accounting documentation No. 094-1/o "The certificate of temporary disability (treatment) of the victim in the road accident" and the instruction concerning procedure for its filling and issue

According to paragraph one of Item 33-1.2 of article 33-1 of the Law of Ukraine "About compulsory insurance of civil responsibility of owners of land vehicles", for the purpose of reduction of regulatory legal acts in compliance with the current legislation I ORDER:

1. Approve form of primary accounting documentation No. 094-1/o "The certificate of temporary disability (treatment) of the victim in the road accident" and the instruction concerning procedure for its filling and issue which are applied.

2. To the Minister of Health of the Autonomous Republic of Crimea, chiefs of managements (head departments) of health care of the regional, Kiev and Sevastopol city public administrations to bring this order to permission of all institutions of health care.

3. Declare invalid the order MZ of Ukraine of 04.08.98 No. 236 "About approval of the accounting form "The Certificate of the Term of Temporary Disability for Submission to Insurance Company", registered in the Ministry of Justice of Ukraine 28.08.98 for 536/2976.

4. To department of the treatment-and-prophylactic help (Hobzey M. K.) provide in accordance with the established procedure submission of this order on state registration in the Ministry of Justice of Ukraine.

5. To impose control over the implementation of this order on the First Deputy Minister of Health of Ukraine Moiseenko R. O.

6. This order becomes effective from the date of its official publication.
