According to article 35 of the Law of Ukraine "About providing organization-legal conditions of social protection of the orphan children and children deprived of parent guardianship" and for the purpose of enhancement of information base on social protection of the orphan children and children deprived of parent guardianship, in particular preserving and protection of their housing laws and interests I order:

1. Approve enclosed:

1.1. Form of the reporting No. 1-ZZhD (annual) "The report on preserving housing laws of the orphan children and children deprived of parent guardianship".

1.2. The instruction for filling of form of the reporting No. 1-ZZhD (annual) "The report on preserving housing laws of the orphan children and children deprived of parent guardianship".

2. Declare invalid the order of the Ministry of Ukraine for family, youth and sport of December 08, 2008 No. 4815 "About approval of the Form of the reporting No. 1-ZZhD" the Report on preserving housing laws of the orphan children and children deprived of parent guardianship "and Instructions for its filling", No. 1251/15942 registered in the Ministry of Justice of Ukraine on December 29, 2008.

3. Department on adoption and protection of the rights of the child (A. Filipishin) to submit this order in accordance with the established procedure on state registration to the Ministry of Justice of Ukraine.

4. This order becomes effective from the date of its official publication.

5. To impose control over the implementation of this order on the deputy minister L. Drozdova.

The instruction for filling of form of the reporting No. 1-ZZhD (annual) "The report on preserving housing laws of the orphan children and children deprived of parent guardianship"

I. General provisions

1.1. This Instruction developed according to the Law of Ukraine "About providing organization-legal conditions of social protection of the orphan children and children deprived of parent guardianship" determines procedure for filling of form of the reporting No. 1ZZhD (annual) "The report on preserving housing laws of the orphan children and children deprived of parent guardianship" (further - the Report).

1.2. The report services for children of the regional, Kiev and Sevastopol city public administrations, district, district public administrations, executive bodies in the cities of Kiev and Sevastopol of city, district councils in the cities constitute authorized body of the executive authority of the Autonomous Republic of Crimea concerning children.

1.3. The report is annually constituted by services for children of district, district public administrations, executive bodies in the cities of Kiev and Sevastopol of city, district councils in the cities and moves to authorized body of the executive authority of the Autonomous Republic of Crimea concerning children, to services for children of the regional, Kiev and Sevastopol city public administrations no later than February 10 of the year following reporting.

1.4. Authorized body of the executive authority of the Autonomous Republic of Crimea concerning children of service for children of the regional, Kiev and Sevastopol city public administrations submit the Report to the Ministry of social policy of Ukraine and Head department of statistics in the Autonomous Republic of Crimea, areas, the city of Kiev and to Management of statistics of the city of Sevastopol no later than February 20 of the year following for reporting.

1.5. The ministry of social policy of Ukraine provides to Public service of statistics of Ukraine summary information on Ukraine and regions no later than March 1 of the year following reporting.

1.6. Text part of the report is filled in Ukrainian. The report is signed by the head and the contractor and sealed organizations, constitutes the report.

1.7. In the report all indicators shall be filled. In the absence of indicator the crossed out section is put.

1.8. The report shall be completed accurately and is legible. Error correction is confirmed by the signature of the head.

1.9. Accounting year - the calendar period which begins on January 1 and comes to an end on December 31 inclusive.

II. Filling of the Section I of the Report "Housing availability at the orphan children and children deprived of parent guardianship and form of their device by the end of the reporting period"

2.1. In this Section the number of the orphan children and children deprived of parent guardianship, form of their device and availability of housing at them is specified.

2.2. In the column 1 of line 01 the total quantity of the orphan children and children deprived of parent guardianship is specified.

2.3. These lines 01 shall equal to the amount of lines 02 and 05 according to all graphs.

2.4. These columns 1 of line 02 shall equal to the amount of data of the column 1 of lines 03 and 04 on all graphs.

2.5. Each of indicators of the column 1 of lines 01-05 shall equal to the amount of these columns 2-8.

III. Filling of the Section II of the Report "Establishment of guardianship over housing which belongs to the orphan children and children deprived of parent care on the property right (use) by the end of the reporting period"

3.1. In this Section the number of the orphan children and children deprived of parent guardianship, (uses) and the number of the guardians appointed over housing having housing on the property right is specified.

3.2. In the column 1 of line 01 the number of the orphan children and children deprived of parent guardianship, (uses) having housing on the property right is specified.

3.3. These lines 01 shall equal to the amount of data of the column 1 of lines 02-06 on all graphs.

3.4. In the column of 1 lines 02, of 03, of 04, of 05, 06 the number of the children having housing on the property right (use) by forms of their device is specified.

3.5. In columns 2-4 of line 01 the number of the appointed guardians over housing which belongs to the orphan children and children deprived of parent care on the property right (use) is specified.

3.6. The amount of these columns 2-4 shall not exceed these columns 1 on everyone to the corresponding line.

3.7. In column 2 the number of the guardians appointed by body of guardianship over housing which belongs to the orphan children and children deprived of parent care on the property right (use), from among physical persons who are guardians, the child's custodians is specified.

3.8. In column 3 the number of the guardians appointed by body of guardianship over housing which belongs to the orphan children and children deprived of parent care on the property right (use), from among persons who are in the family, related relations with wards, but not their guardians, custodians is specified.

3.9. In column 4 the number of the guardians appointed by body of guardianship over housing which belongs to the orphan children and children deprived of parent care on the property right (use) is specified, from among the faces which are not consisting in the family, related relations with wards and is not their guardians, custodians.

IV. Filling of the Section III of the Report "Condition content of housing which belongs to the orphan children and children deprived of parent care on the property right (use) by the end of the reporting period"

4.1. In this Section the number of the orphan children and children deprived of parent guardianship who possess housing on the property right (use) which is in condition, suitable or not suitable for accommodation, is specified.

4.2. In the column 1 of line 01 the number of the orphan children and children deprived of parent guardianship who possess housing on the property right (use) is specified. This indicator shall equal to indicator of the column 1 of line 01 of the Section II of the Report.

4.3. These columns 1 of lines 01-03 shall equal to the amount of these columns 2-6 of these lines.

4.4. In the column 1 of line 02 the number of the orphan children and children deprived of parent guardianship who possess housing on the property right (use) which is in condition, suitable for accommodation, is specified.

4.5. These columns 1 of line 02 shall equal to the amount of these columns 2-6 of this line.

4.6. In the column 1 of line 03 the total quantity of the orphan children and children deprived of parent guardianship who possess housing on the property right (use) which is in condition, not suitable for accommodation, is specified.

These columns 1 of line 03 shall equal to the amount of these columns 2-6 of this line.

4.7. These lines 01 shall equal to the amount of these lines 02 and 03 according to all graphs. V. Filling of the Section IV of the Report "Housing availability at orphan children

and the children deprived of parent guardianship and providing them with housing after attainment of majority"

5.1. In this Section the number of the orphan children and children deprived of parent guardianship, reached age of majority is specified, are provided with housing, being on room accounting or received housing for the accounting period.

5.2. In the column 1 of line 01 the number of the orphan children and children deprived of parent guardianship, reached age of majority for the accounting period is specified.

5.3. These lines 01 shall equal to the amount of these lines 02-05 according to all graphs.

5.4. In columns 6 and 7 of line 01 the number of the orphan children and children deprived of parent guardianship who received housing during the accounting period is specified.

5.5. In the column 6 of line 01 the number of the orphan children and children deprived of parent guardianship who received premises is specified.

5.6. In the column 7 of line 01 the number of the orphan children and children deprived of parent guardianship who received housing from fund of social appointment is specified.
