1. Approve:

1.1. The procedure for creation and representation of requests for receipt of public information (is applied).

1.2. Form for representation of request for receipt of public information from the physical, legal entity, from consolidation of citizens without the status of the legal entity in writing and e-mail it (is applied).

2. To provide to management of control and clerical work submission of this order on state registration in the Ministry of Justice of Ukraine in the procedure established by the legislation.

3. To provide to the sector concerning interaction with mass media and the public placement of this order on the official website of the Ministry of infrastructure of Ukraine.

4. This order becomes effective from the date of its official publication.

5. To impose control over the implementation of this order on the deputy minister - the chief of staff of Korniyenko V. V.
