1. Make changes to the Regulations on the Unified state register of enforcement proceedings approved by the order of the Ministry of Justice of Ukraine of May 20, 2003 No. 43/5, registered in the Ministry of Justice on May 21, 2003 for No. 388/7709 (with changes), having stated the paragraph of the sixth of Item 2.3 of Chapter 2 in such edition:

"full name of the claimant and debtor, their address and, in the presence, - identification code on EGRPOU for legal entities or registration number of accounting card of the taxpayer for physical persons (series and passport number - for physical persons which because of the religious beliefs refuse adoption of registration number of accounting card of the taxpayer and officially notified on it relevant organ of the State Tax Service and have mark in the passport);".

2. To department of interaction with authorities of the Ministry of Justice of Ukraine (Zerkal O. V.) to provide submission of this order on state registration according to the Presidential decree of Ukraine of October 03, 1992 to No. 493 "About state registration of regulatory legal acts of the ministries and other executive bodies" (with changes).
