According to article 19 of the Law of Ukraine "About access to public information", article 11 of the Law of Ukraine "About obligatory national social insurance in connection with the temporary disability and expenses caused by burial" board of Social Insurance Fund on temporary disability DECIDES:
1. Approve form for representation of request for receipt of public information, the Procedure for creation, representation of request for public information and its obtaining in Social Insurance Fund on temporary disability which is applied.
2. To determine management of organizational and documentary support of Executive directorate of Fund responsible for study, systematization, the analysis and control of satisfaction of requests for public information and provision of consultations during registration of request.
3. Responsible for the organization of work on consideration of requests for public information the deputy director of Executive directorate Fonda who according to distribution of obligations coordinates this management is determined.
4. To submit to executive directorate of Social Insurance Fund on temporary disability this resolution on state registration to the Ministry of Justice of Ukraine.
5. This resolution becomes effective from the date of its official publication.
